Referrals: Data Elements

Thank you for your interest in the HIFIS Improvement Project about Referrals and Service Matching.

This is survey 2, covering required data elements connected to referrals.

For the purposes of this survey, a “referral” is referring to any time when you have a client and you arrange for them to receive services at another organization. This could be a simple referral (“here’s the address/phone number”) or a warm transfer (“I’m introducing you to your new caseworker”). It could include a service hand-off (“you’re moving into supportive housing”) or a supplemental service (“let me connect you with the health clinic”). Prioritization decisions are included as referrals (“we’ve agreed that this client will be matched to this housing resource”).

A shelter admission is a service. Providing case mangement is a service. Feeding someone is a service. A housing subsidy is a service. Right now, referrals in HIFIS are part of another service. Should they be counted as services in their own right?
