Referrals: Current State

Thank you for your interest in the HIFIS Improvement Project about Referrals and Service Matching.

This is survey 1, covering the current state of referrals in HIFIS and the need for change.

For the purposes of this survey, here are some definitions:

  • A “referral” is referring to any time when you have a client and you arrange for them to receive services at another organization. This could be a simple referral (“here’s the address/phone number”) or a warm transfer (“I’m introducing you to your new caseworker”). It could include a service hand-off (“you’re moving into supportive housing”) or a supplemental service (“let me connect you with the health clinic”). Prioritization decisions are included as referrals (“we’ve agreed that this client will be matched to this housing resource”).

  • “Tracking referrals” means any information related to the referral process that you want/need to collect.
